When can I cancel my Swiss health insurance?

The deadlines for basic insurance, supplementary insurance, deductible and insurance model — including the dates for switching on 1 January 2027.

In brief

You can cancel your mandatory basic insurance (KVG/OKP) for the end of the year once your insurer has notified you of next year's premium: the cancellation must reach your insurer by the end of the month before the new premium takes effect — for a switch on 1 January 2027, that is expected to mean Monday, 30 November 2026.

There is also an ordinary cancellation right at the end of each calendar half-year with three months' notice: to leave on 31 December, your letter must arrive by 30 September; to leave on 30 June, by 31 March. The June exit is only available if you have the standard deductible of CHF 300 and no special insurance model.

Supplementary insurance (VVG) runs separately under its own contract terms: since the VVG revision you can cancel at the end of the third policy year, and annually thereafter, with three months' notice — some contracts set longer periods.

Basic insurance: the two ways to cancel

Swiss health insurance law provides two exit routes from basic insurance (Art. 7 of the Health Insurance Act, KVG). First, ordinary cancellation: at the end of a calendar half-year — 30 June or 31 December — with three months' notice. Second, cancellation after the premium notification: your insurer must inform you of the newly approved premium at least two months before it applies (in practice by the end of October) and point out your right to switch. From that notification, you can cancel with one month's notice for the end of the month before the new premium takes effect.

For the current switching season, the dates are:

  • Switching on 1 January 2027 after the premium notification: your cancellation is expected to have to reach the insurer by Monday, 30 November 2026 — the Federal Office of Public Health confirms the binding dates in its fact sheet for the new premium year (expected at the end of September 2026).
  • Ordinary cancellation for 31 December 2026: arrival by 30 September 2026.
  • Switching on 1 July: arrival by 31 March — only possible with the standard CHF 300 deductible and no special insurance model.

Arrival counts — not the postmark

What matters is the day your cancellation arrives at the insurer, not the postmark. The Federal Office of Public Health recommends sending the written cancellation by registered mail or A-Post Plus by mid-November, so you have proof of delivery. Changing your deductible or insurance model

Deductible (franchise) and model changes follow their own deadlines (Art. 94 and 100 of the Health Insurance Ordinance, KVV):

  • Raising your deductible: only from the start of a calendar year; written notice to your insurer by 31 December.
  • Lowering your deductible: only from the start of the next year; the notice must reach your insurer by the end of November.
  • Leaving a managed-care model (family doctor, HMO, telemedicine): only at year-end, notice by the end of November.
  • Joining a managed-care model: possible at any time.

Which deductibles exist?

The standard deductible for adults is CHF 300 per calendar year; the optional higher levels are CHF 500, 1'000, 1'500, 2'000 and 2'500. Children have no standard deductible; optional levels run from CHF 100 to 600. A proposal to raise the minimum deductible is currently only in the legislative process — nothing has been decided, and CHF 300 continues to apply.

Supplementary insurance: separate rules

Basic and supplementary insurance are legally separate. When you switch basic insurers, your current insurer may neither force you to cancel your supplementary policies nor cancel them itself just because you are leaving (KVG Art. 7 para. 7 and 8).

Supplementary policies fall under the Insurance Contract Act (VVG): since the 2022 revision, you can cancel any policy at the end of the third policy year and annually thereafter, with three months' notice, in writing or in text form (VVG Art. 35a). Your exact notice periods are in your policy conditions — some contracts set up to six months.

From our advisory practice: only cancel a supplementary policy once the new insurer has confirmed your acceptance in writing. Unlike basic insurance, a supplementary insurer may ask health questions, impose exclusions or decline your application. For basic insurance, admission is mandatory: every insurer must accept you unconditionally, without waiting periods and without health questions.

What else to know

You cannot end up uninsured: by law, your old basic-insurance relationship only ends once the new insurer has confirmed seamless coverage to the old one (KVG Art. 7 para. 5).

If you have unpaid premiums or cost shares that have been formally reminded, you generally cannot switch insurers until they are settled; the law provides for limited exceptions (KVG Art. 64a).

Based on the pattern of previous years, the 2027 premiums are expected at the end of September 2026; your insurer must notify you of your new premium by the end of October. That notification opens the switching window.

Does the postmark count?

No. The decisive date is when the cancellation arrives at the insurer. The Federal Office of Public Health therefore recommends registered mail or A-Post Plus, sent by mid-November.

Can I cancel only my supplementary insurance and keep the basic insurance?

Yes. Basic and supplementary insurance are separate contracts and can be cancelled independently — including with the same insurer, and in either direction: your insurer may not cancel your supplementary policy just because you switch basic insurance.

Does the new insurer have to accept me?

For basic insurance, yes: admission is mandatory — unconditional, without waiting periods and without health questions. For supplementary insurance there is no such obligation; health questions, exclusions or a rejection are permitted.

When can I change my deductible?

You can choose a higher deductible for 1 January with notice by 31 December. A lower deductible also takes effect at the start of the year, but the notice must reach your insurer by the end of November.
